What happens after I fill out the Candidate Form?

After we receive the Candidate Form we will send you back an e-mail with instructions on next steps and prices.

• If you are located locally you will be scheduled for an in-office consultation visit. During this visit you will be required to bring in a recent (within the past 2 years) MRI.
• If you are located out of town you will complete an online medical history form and will be asked to mail us a recent (within the past 2 years) MRI. Once these are received you will be scheduled for a phone review with one of our physicians.

More detailed information will be provided in the e-mail sent to you once you submit the Candidate Form.

Prolotherapy

Mild to moderate muscle, ligament, and tendon tears, joint arthritis, degenerative joint conditions, torn spinal discs

Platelet Lysate

Patient Information:

IMPORTANT – Tardiness and Same Day No-Shows
We request that you arrive at least 15 minutes prior to your scheduled appointment time in order to complete your registration and any other paperwork that may be required.
Please contact the office if you are running late. Patients arriving more than 30 minutes late may be asked to reschedule. Those who cancel without a 24 hour notice, or do not show up to their scheduled appointment without notifying the office will be assessed a $75 administrative fee.
If our office is responsible for a delay, your session will be completed in its entirety.
Billing / Insurance
StemCell ARTS procedures are not covered by insurance at this time and are self-pay. However, if you are located locally, your in-office consultation visit can be filed on your medical insurance.
StemCell ARTS accepts checks, and the following credit cards: Visa, MasterCard, American Express & Discover. We do not accept cash payments for medical services or co-payments. If you need to contact our billing department, please call 1-866-263-8525.
You may notice Physical Medicine Associates is listed on your billing statements. This is StemCell ARTS’ formal billing name.
